What is filgrastim and why is it used in IVF?
Filgrastim is a synthetic form of granulocyte colony-stimulating factor (G-CSF), which is a naturally occurring protein produced by our bodies. It has been used safely in oncology for many years to stimulate the production of white blood cells in people undergoing cancer treatment. More recently, fertility researchers have explored its role in supporting the uterine environment during IVF.
In fertility treatment, filgrastim may be considered for patients experiencing:
· Recurrent implantation failure, where high-quality embryos have been transferred multiple times without successful implantation
· Thin or unresponsive endometrium, where the lining does not reach the ideal thickness despite standard oestrogen support
Some studies have explored whether G-CSF may influence uterine blood flow and support increased endometrial lining thickness, however findings are variable and not conclusive. Several studies and reviews published in 2025 [1, 2] have shown encouraging improvements in implantation rates, clinical pregnancy rates, and live birth rates.

What does filgrastim treatment involve?

· In select cases, a carefully timed intrauterine infusion, usually around the embryo transfer window
